Transitional Recovery Housing Program
A phased housing model designed to support long-term stability and employment.
Rise Together Project operates a structured recovery housing continuum designed to move men from high-barrier stabilization into independent, permanent housing. Our model emphasizes accountability, employment readiness, and measurable progress.
Phase One: Stabilization Housing
Men exiting treatment, incarceration, or unstable living situations begin in structured recovery housing focused on rebuilding stability and routine.
• Substance-free shared housing
• Weekly goal-oriented check-ins
• Employment navigation and job placement support
• Documentation recovery and financial planning
• Coordination with probation and treatment providers
Phase Two: Transition Housing
Residents demonstrating sustained recovery and employment stability may transition into a lower-density step-down residence designed to prepare them for independent housing.
• Defined 6–12 month lease period
• Ongoing employment requirement
• Weekly case management
• Budgeting and savings planning
• Independent housing plan development
This program is designed to increase employment retention, extend housing stability, support successful exits to independent rental housing, and reduce returns to homelessness or incarceration.
